14–40 Day Water Fasts: Healing Power, Benefits & Risks

Why would anyone fast for two weeks—or more?
For those seeking the deepest levels of physical, emotional, or spiritual healing, a 14–40 day water fast may offer what shorter fasts cannot. Once your body has fully adapted to ketosis, longer fasts allow for profound detoxification, immune recalibration, trauma release, and sometimes even the reversal of chronic illness. However, they also require care, preparation, and guidance.

In this article, you’ll learn what happens in extended fasts, how to prepare and refeed, and how to distinguish between a true healing crisis and signs that it’s time to stop.

Note: Extended water fasts of 14–40 days are powerful but demanding. They should never be undertaken without expert supervision, especially if you have chronic health issues, take medications, or are unsure of your body’s limits. Why in the world would you ever want to fast for two weeks or more?

If only a tiny percentage of people in the Western world ever undertake a water fast of any kind, then only a tiny percentage of people who water-fast will ever undertake an extended healing fast of 14 days or more. Why? There’s simply no need. For everyday cleansing and healing, occasional 7-10 day fasts combined with regular short fasts such as the 3-day or 36-hour water fast should keep you in excellent health.

Benefits of extended water fasts (14-40 days):

However, in order to reach the deepest possible level of healing and reap the greatest benefits, it is necessary to dig deeper with a longer fast. For instance, certain serious physical illnesses – those often deemed incurable by Western medicine – require the cleansing of an extended fast in order to permanently heal.

Despite what most modern medical doctors may tell you, conditions as wide and varied as Long Covid, gut issues (such as IBS, ulcerative colitis and food intolerances) adrenal issues (CFS/ME), diabetes, chronic high blood pressure, autoimmune disorders (including allergies and asthma), as well as certain types of tumors are all potentially curable. I know this for a fact, because I have personally worked with clients suffering from ALL the health issues listed above to the point of a full healing.

As a water fasting coach, I’m privileged to witness this miraculous healing potential of the human body every day! It does take time, though, and in order to heal effectively from chronic illness, the length of the fast must typically last 14-40 days.

In helping clients decide how long to fast, three main factors come into play.

First is the overall symptom picture – that is, the sum total of active health issues, all of which constitute a toxic load contributing to illness. Although, for example, someone may wish to fast in order to heal Long Covid, further questioning almost invariably reveals other health issues in the background, both past and present. Whether presenting with symptoms of their own or whether entirely sub-clinical and ‘invisible’, these apparently secondary issues tell a story of how, over the years, a client has arrived at the present moment.

The second factor which suggests how long a client may need to fast is the strength of their immune system. The primary factors here are age, diet, stress and, in the case of chronic illness, how much the immune system has already been worn down in fighting a war of attrition.

The third factor concerns the degree of prior cleansing – and especially water fasting. Quite simply, the more you’ve already fasted, the more quickly and powerfully your body can mobilise your immune system for healing.

Physical, emotional, spiritual healing:

Beyond physical healing, the deepest emotional and spiritual cleansing can similarly take place only through the sheer length of an extended fast. In fact, what I’ve seen over many years of coaching is that in order to achieve the greatest physical healing, it’s also necessary to surrender emotionally and spiritually to the whole process. A big part of my job is to help facilitate that process.

Although nowadays we tend to remember only the Biblical 40-day fasts of Moses and Jesus, the fact is that many spiritual traditions over the millennia have demanded 40-day water fasts. Even Pythagorus required potential students to undertake a 40-day fast before he was willing to accept them. As much as we balk at the idea of giving up food for such a long time, it’s mostly just a question of unwillingness to forgo the addictions and pleasures of life. Don’t believe the voice of your fears and reluctant ego. You won’t starve to death.

Unless you’re seriously malnourished and underweight to begin with, you carry the better part of 100,000 calories on you, locked in your fat tissue and waiting to be released through ketosis. That’s enough to last you well over 40 days. If you’re overweight, you could potentially fast for much longer (although in most cases this is not advisable).

In other words, getting through a 14, 21 or 40-day fast is often a mind game more than anything.

Letting go of your fears about fasting is just the beginning. The process of surrender also eventually leads to the release of pent-up, self-destructive emotions and, for some people, a catharsis of trauma from the distant past. In doing so, you’ll free yourself: discovering a place of inner calm and spaciousness where you can finally breathe. This is a hugely important part of the healing process.

Experiencing a healing crisis:

Although in one respect an extended fast simply continues the notion of a 7-10 day water fast, it is also much more powerful because it gives you the opportunity to experience deeper ‘healing crises‘. As described in the article on the 7-10 day water fast, a healing crisis often occurs towards the end of the first week of water fasting, as the symptoms of old illnesses, injuries and traumas resurface, before being permanently expelled from your body.

A similar process often occurs around the end of the second week of fasting – and this is the reason that if you decide to extend a 7-10 day water fast, it’s worth aiming for at least 14 days. This second healing crisis tends to call forth deeper issues than the first healing crisis, or, alternatively, finishes resolving those issues which were not fully cleared during the first healing crisis. In other words, it’s from the beginning of the second week of water fasting that your body can begin to heal from more serious health issues. Simply, up until this point, your body has been focussed on cleansing the toxins of everyday life (and especially so if you haven’t been fasting regularly).

For the deepest and most serious health issues, healing crises often occur much later into the fast, whether 20, 30 or even 40 days. There’s no way to accurately predict when they will occur. You can only trust your body and let nature take its course…

Occasionally, healing crises can be extremely intense, especially when they occur late into a fast. When this happens, it’s critically important to be able to tell the difference between a healing crisis and a sign from your body urging you to stop the fast. This is where working with an experienced water fasting coach can really help. If it’s a healing crisis, you should ideally try and push through. If it’s not, you should stop immediately! Not every healing process is accompanied by a dramatic healing crisis. Sometimes symptoms of illness and trauma simply begin to disappear. In cases like this – when no clear sign indicates that you’ve obviously freed yourself from a health issue – it can be difficult to know when to end the fast. This is another reason why it’s advisable to consider undertaking an extended fast with the help of an expert who understands the subtleties and symptomatology of fasting. The other main reason for working with a professional is to make sure you don’t overstep your body’s nutritional capabilities, as a prolonged fast begins to reach its physical limits.

Complete a 3-day water fast

The dangers of fasting too long:

  • Depletion of fat (adipose tissue)
  • Depletion of protein (amino acids from muscle tissue)
  • Depletion of electrolytes

If you continue fasting indefinitely there comes a point where the fast turns into starvation. You obviously don’t want to overstep this mark! For when your fat stores are finally depleted, the body has no choice other than to devour muscle tissue, as well as feed from your inner organs. You’ll do yourself serious damage. Fortunately, though, the body sends a clear sign: extreme hunger. Although it’s unlikely you’ll miss this red flag, it’s also true that normal periods of hunger can develop during an extended fast. The urban myth out there on the internet – that hunger simply disappears on the third day – is a gross oversimplification of what actually happens. On longer fasts, it isn’t uncommon for my clients to believe that ‘true hunger has returned’, and that they should break their fasts. Usually (but not always), this phenomenon simply constitutes a totally normal physiological phase of the fast, or – even more likely – their digestive juices are being driven by an increasing emotional desire to eat. Without someone offering an objective mirror on the outside, it can be extremely difficult to distinguish between the different possible scenarios!

In addition, two other less obvious scenarios demand the end of a fast. The first possibility is that you run out of amino acids (derived from muscle tissue) before you deplete your fat stores. In order to power your body as a whole, it’s true that ketosis is extremely efficient. The problem is that the brain demands another fuel entirely: glucose. And this cannot easily be metabolised from fat tissue. Instead, the body must extract it by breaking down proteins in a process called gluconeogenesis (described in greater detail in my article on the 3-day fast). However, the actual danger of depleting amino acids before fat tissue is largely theoretical. I have never witnessed this happen myself, and it is only likely to arise on fasts longer than 40 days in the case of someone who is morbidly obese with severely atrophied muscle tissue.

The final possibility is certainly a real danger: the depletion of electrolytes (blood salts), especially potassium or sodium. Although unlikely, this process is potentially life-threatening! Understandably, many people decide to take electrolytes while fasting, precisely in order to avoid this scenario. If the sole reason for fasting is to lose weight, then by all means do so! However, if you are fasting because of a chronic health issue, it is critically important to understand that taking electrolytes during a water fast compromises both the deepest cleansing and healing – as I have observed clinically among clients who choose to fast using electrolyte supplementation. (I will delve into the physiology of this in a future article.) Although the danger of electrolyte imbalance usually increases from only about the 21-day mark, it is extremely important to respond to the first warning signs if and when they do arise. Unfortunately, this is difficult to the untrained eye, because the symptoms of potassium and sodium depletion overlap with other totally normal detox symptoms. The key is in understanding the context as well as the overall, underlying symptom picture.

Refeeding:

After you break an extended fast, it’s extremely important to follow a well structured meal plan. In an ideal world, this should be tailor-made to the individual person, based on prior nutritional reserves, any dietary sensitivities, as well as the given health issue for which someone has fasted. How to break a water fast?

If you return too quickly to a normal diet, you risk encountering both digestive problems as well as ‘refeeding syndrome’. This is a potentially fatal complication caused by the change from ketosis back to your everyday metabolism.     im trying to cure RA.
    Last water fast I did was 7 days and I got a lot of chest pain and heart palpitations and shortness of breath. A week later I tried a dry fast and after 50h I got the same problem so I quit.
    My 3 question are about combined fasting, 2-3 days dry fast+water fast of 10-14 days like dr Filanov recommends.
    1. Should I try to endure through those symptoms?
    2. Can the faster onset of ketosis due to dry fasting help with getting faster to the first crisis and maybe make it less hard to endure?
    3. Can doing a 36h dry fast every weekend for 1-2 months help the body clear enough so that doing a longer water fast of 14+ days will be easier?
    Im on a high raw vegan diet.

      Hi Karol,
      Thanks for writing. Given your multiple questions combined with chest pain / cardiac symptoms on previous fasts, it sounds like this is something we should explore more properly in a full consultation. Your symptoms may be totally harmless, but I’m afraid it would be irresponsible of me to respond superficially here when there are potential safety risks in the background. The one thing which will ultimately kill you while fasting is your heart, whether through electrolyte imbalance, pathologically low blood pressure or – for those who already suffer from serious cardiac illness – the extra load on the heart caused by detox. Since you mention Sergei Filanov, you’re probably already aware that one of his ‘red flags’ for stopping a dry fast is cardiac symptomatology caused through excessive loss of fluids. (This is an issue which should not impact on extended water fasts.)

      Except for those who already suffer from chest pain caused by acute and chronic Covid, ‘a lot of chest pain’ is not a normal symptom – and certainly not within the first 50 hours! In order to continue fasting safely, it would be extremely important to understand why you’re experiencing these symptoms!

      In the case of a full consultation, I would also want to investigate your diet in relation to RA. Given my clinical observations over the years with numerous clients suffering from RA, there may be a connection to your symptoms: both in everyday life as well as while fasting.

  2. Can fasting help with large liver cysts? Eg 10cm +. If so how long would you recommend?

      Thanks for writing. Fasting can definitely heal cysts anywhere in the body. However, in this case it’s not as simple as recommending a specific particular length of fast. Cysts tend to be a symptom of chronic metabolic dysregulation: something which most likely has arisen as a result of many years, during which multiple stressors have gradually forced the body away from optimal homeostasis. To this extent, it’s important to understand the whole process in order to give insight towards the best fasting protocol. This is why, when I’m working with a client, the first thing we do is look at the full case history.

      Without going into your own case history, what I can tell you is that you probably need to be looking at a fast of at least 21 days, or even a series of fasts to address the root issues which have caused your own dysregulation. Other factors which play into the equation are any additional cleansing protocols you may have tried up until now, your diet over the last 6 months or so, and current stress levels. If we were working together on a daily basis through your fast, I would probably also recommend using occasional targeted periods of dry fasting within the overall water fast. However, this is not advisable for people fasting without expert guidance, precisely because the detoxing power of a dry fast can sometimes be overwhelming in cases of cysts.

  3. Hi Tallis, I’ve diagnosed with bladder cancer (muscle invasive T2 – 5cm diameter) in Jan 2022, and my journey since has been: chemo and radiation during that same year. Recurrence happened 9 months later in June 2023, although much lighter case (T1) in few spots – to help in laymen terms Dr. called it “surface rust” and removed it (TURBT). Next came BCG treatments with recurrence in 7 months, although this time it was labelled as “residual” cells found in one small spot. As it continues to recur, I’ve started obsessive reading on how to strengthen immune system which led me to start on full Plant Based diet/Vegan and organic (as much as possible) in August 2023 after reading of animal proteins and casein in dairy being promoters of cancer growth. Vegan now for roughly 1yr 3m now I have another spot found during my last cystoscopy with another TURBT scheduled in a couple of weeks (December 3rd). Overall, the trend has been positive from the perspective size – meaning each recurrence is smaller and smaller (last one “residual” cells), but this is just not going away so I’m looking to up the game on helping my immune system to work in concert with the treatment by the doctor. So I came across many different nutrients that I’ve been increasing in my diet (tumeric, ginger, wormwood, barley grass, spirullina, dandelion etc…) which I’m administering through smoothies, and couple of them through bought supplements… more so with smoothies rather than supplements using blueberries and blackberries (organic). Additionally I came across fasting a while back and did my first fast at 96 hours. I tolerated the 4 day fast quite well, and actually felt great after 2nd day. and frankly I could have went longer with no problem. I’m getting ready for the next fast which I planned to be 7-10 days, and I came across your website today while researching how to do this properly. I spent last 3 hours reading a lot of your articles, watching your videos and reading all your answers to comments and questions from others (obsessive reading marathon) and it seems the benefit of healing which I seek I would need to get into more extended fast period 14+ days or longer.
    Considering that in 3 weeks (December 3rd) I’m going in for the next TURBT surgery, would it be advisable to undertake that longer Water Fast before surgery? Or better wait until after surgery – and how long after?

You’re definitely going to have to push well beyond your current longest fast in order to start making inroads, but this is a complicated case which even involves time restrictions and the element of surgery.   4. Can fasting heal receding gums. Im currently on day 14 and dont see any changes yet.

      Thanks for writing. I’ve had some clients report that their receding gums have improved. In ascertaining the likelihood of this, though, it’s also important to understand the reasons for this happening specifically to you – and of course, this would require a much more detailed conversation. What I can say to you is that very often deep dental healing starts to take place in the third week of a fast (most typically towards the end of the week). So, considering where you are in your own fast, it’s not that surprising you’ve not seen any changes yet. Hang in there!

  5. I am on my 6th day of a 30 day fast. I have had very uncomfortable reflux that last 2 days. I don’t normally deal with it when I’m eating. Any suggestions? Thank you.

      Thanks for writing, and apologies for the delay in responding. Your comment fell between the cracks and I saw it only now! Acid reflux is an issue for many people on extended fasts. When coaching clients, it’s something I often work with on a daily basis because it can be so hard to control. There can be many causes (which is the first issue to identify!), but based on the information you gave, it sounds to me like this was most likely a consequence of increasing acidic load due to detox. All end-products of detoxification are acidic, which lowers blood pH. When the kidneys are working to capacity and unable to rebalance this, the body often adopts a strategy of turning blood acids into stomach acid: hence acid reflux.

    Hello,
    i am currently on a 14 day fast, im at day 10, id have to admit that im very tired physically, my brain is functioning exceptionally but the body just feels overwhelmed, i noticed that yesterday when i added electrolytes (coconut water) to my routine i felt amazing so i suspect im experiencing some measure of depleted electrolytes, either way im pushing through 4 more days before i enjoy a nice bowl of home made broth to break my fast. Id love to push to 21 days but not this time around perhaps next time. I endorse fasting, the benefits are wonderful but it requires a will power that cannot easily be found if you are addicted to the pleasures of eating. I highly recommend fasting, ive never felt better mentally and my skin is extremely clear, ive lost some pounds too. Ive done this fast to reset my body as i am about to change my eating habits.

      Thanks for sharing. That’s great you’re experiencing the healing benefits of fasting! Just wanted to jump in here about adding electrolytes. I believe you when you say that you felt amazing by adding the coconut water. The problem is that electrolytes significantly reduce the cleansing and healing potential of any pure water fast. I can guarantee you that at Day 10 there’s no way (with @99.5% certainty) you were suffering an electrolyte deficiency – it was most probably simply deep detox, and that’s a good thing!

I cannot seem to find anything about it online…at least not from a trusted source. Was there a particular resource you rely on for this? Thanks so much! I’m a huge fan of Gandhi AND of water fasting, so I’d love to learn the history of the story!!

I simply received the story through word of mouth – and, like you say, nowadays it’s important to find a trusted source.     Hello Dr. Tallis,

    I have gone through 4-6 3 day water fasts successfully over the years. Recently, I attempted on very short notice to go for a 21 day fast. I had been planning a 10 day religious fast with my church and essentially I just jumped on it early and was planning to go a few days after. I was pursuing growing spirituality and bodily healing for chronic, severe acid reflux (LPR specifically). I aborted the fast at the end of 6 days.

    My prior attempt at a 10 day fast was a year ago, and I experienced what I believe was electrolyte deficiency (faintness, headache, heart palpitations, “stars” or “pinpricks of light” in my vision) in the middle of day 3 and I became fearful and aborted the fast. This experience led me to search around and convinced me that electrolyte supplementation was necessary. It matched with my latest experience, but it did not quite sit right with me, as I believe it is unlikely that all humans in history who have undertaken fasts were carrying around a bag of salt and eating it throughout.

    I read in this article that electrolyte supplementation is not only not necessary, but detrimental for deep healing (which was one of my main goals). Ultimately, I ended the fast early at 6 days, because I had severe doubts about the way I prepared and was executing the fast. It seems that one of my main mistakes has been following the “mind” and drinking water in excess of thirst, to fill a “number”. I believe this likely contributed to the electrolytes being washed out (if that is what occurred).

    I plan to undertake a 21 day fast at some point, and I may likely book a consultation when I plan to do so! — I was curious if you believe water fasting can cure ailments such as GERD/NPR? — I am not overweight by any standard, and I do largely follow a clean, whole foods diet (with exceptions), but I am thoroughly convinced that the medical community has nothing but poison to offer on this subject. My symptoms have been severe enough that I have developed chronic lung inflammation (not affecting my ability to do most things, but certainly not in bodily peace and wellness).

    This is a long comment, and if you read this far, I greatly appreciate it! I resonate with the idea of letting thirst drive water consumption, and that the body is much more capable than people believe, without supplementation. My wife and I have 2 toddlers and a 3rd baby on the way, so it is not exactly a “restful” environment to fast, but I am committed to finish it nonetheless. Thank you for the articles, and I hope to complete an extended healing fast in the next year!

Without having worked together through your fast, it’s hard to say exactly what caused your symptoms last year, but electrolyte depletion is highly unlikely over such a short timeframe. In terms of healing from GERD / acid reflux, I’ve worked with numerous clients who reliably heal from this issue through an extended fast. The most important thing to know is that if you suffer from this issue in everyday life, then there’s a much higher chance of it developing and deteriorating in the first week of your fast, so it’s extremely important to respond to any symptoms in real time.     2. According to Dr. Berg, GERD/Acid Reflux is often caused by stomach acid not being acidic enough to cause the sphincter at the base of your esophagus to engage fully. He has suggested taking Betaine HCL with meals to increase your stomach acidity.

        Thanks for sharing. You’re absolutely right about the cause of acid reflux in everyday life. In fact, low stomach acid lies quietly behind many people’s health issues, from digestive disorders like IBS and SIBO to allergies, asthma and even autoimmune problems. Especially for someone who already has a weakened LES (lower esophageal sphincter), the increased acidity during a fast exacerbates an already existent problem.

        Using betaine hydrochloride in everyday life to increase stomach acidity can work well as a temporary means to address the symptom, but the most effective method to fully heal this issue – and quickly – is to do an extended fast! I know this may sound contradictory, if fasting makes reflux worse during the fast itself… However, fasting ‘tunes up’ all the organs of the body and recalibrates hormonal and enzymatic functions, including how the stomach finds its homeostasis. Most of my clients feel the improvement after the fast, once the refeeding period is complete.

  10. Hi I found this to be very insightful as you address many things others miss. What is your stance on healing autism through fasting? My son is now 20 verbal and high functioning after the past 17yrs of alternative healing (previously severe non verbal). Home sauna, massage chair, lymphatic work, juicing, (tried most diets til we tailored our own) supplements, herbs, therapy, full moon protocols…

    We adapted to fasting when he was 6 (juice only) and began our 1 day a week from age 9-14. He has completed several juice cleanses up to 7 days and currently on day 13 of his 1st 28 day juice cleanse. I have done many short juice cleanses and 5 extended juice cleanses (30+ days) and several short water fasts less than 10. I did one 17 day water and one 24 and was severely sick after both. I just completed a 67 day juice cleanse and took a 3 week break and started my current water fast and on day 7.

    This is the 1st time not feeling excessive pain and nausea so I feel the prior cleanse cleared the path for healing. I’m working towards healing cervical cancer with my prior cleanse would 40 days still be my goal? I want to follow the same path with a short break than a water fast for my son. Does he need to focus on a 40 day, has anyone ever healed autism with a fast? Tia

      Thanks for writing, and apologies for the slow response – I’m in the middle of moving house! I’m glad you and your son are experiencing the benefits of juice cleanses and water fasts. In terms of permanently healing autism with a fast, I’ve not heard of this and neither have I worked with any clients who have attempted this. What I would say is that appropriate diet and fasting can be very powerful tools in managing symptoms, but ultimately the emphasis probably needs to be on long-term diet more than anything else. In terms of healing cervical cancer, it’s very difficult for me to throw around any simple number without working together more closely, but there’s no question that your prior juice cleanses will currently be contributing to a significantly more powerful water fast as a result!

  12. I am a 44 year old woman in the middle of an extended fast. So I was reading articles about water fasting and stumbled upon your page. I am trying to heal my body. I have PCOS, insulin resistance, and possibly ovarian cysts. I thought that the right thing to do was to take electrolytes. But now I read here that could reduce my healing. I’m on day 14. My plan is to go at least 21 days, as long as there are no signs that I should stop. Then at 21 days I will reevaluate whether to continue to 40 days. Thank you for the information shared here. It definitely provides me with hope on my journey.
